Introduction:The present study aims to provide an overview of the pharmacokinetics, pharmacodynamics, and drug interactions of Tadalafil tablets and the drugs that they might affect in patients with erectile dysfunction (ED).
Methods:Sildenafil hydrochloride (Viagra) is one of the active ingredients of the generic Tadalafil tablets.
Results:In a single-center, double-blind, placebo-controlled, randomized, crossover study, the pharmacokinetics (Cmax, Cmin, C1/2, AUC) and pharmacodynamics (t½, AUC(max)) of Tadalafil tablets were assessed before treatment and after administration of the tablets (2.5, 10, 20, 30 and 40 mg). The pharmacokinetics of Tadalafil tablets were assessed in healthy men (age: 35-45 y; range: 21-50 y; n=9), as compared to men with other sexual disorders, and in men with ED (age: 45-45 y; range: 21-50 y; n=9). Tadalafil tablets were well-absorbed, with no significant pharmacokinetic interaction between Tadalafil and a single oral dose of 10 mg (20 mg) administered once daily. The mean Cmax of the Tadalafil tablets was increased with higher doses (20 mg) of Tadalafil (0.25 mg). Tadalafil was not associated with a decrease in plasma half-life or mean volume of distribution at steady state (median, 0.12 mL/kg/hour; range, 0.04-1.15 mL/kg/hour). Tadalafil was associated with a significant increase in AUC, Cmax, Cmin and AUC(1-0.25, 1.00-1.15, respectively), and the AUC(0-0.25, 1.00-1.15) were similar to those observed in healthy men with ED (0.25 to 2.0 mL/kg/hour; range, 0.05-1.00; P(>interaction) > 2.0, respectively). Tadalafil could not be associated with any clinically significant pharmacological effect, and the pharmacodynamic effects of Tadalafil were not influenced by sexual activity. In men taking a low-dose, continuous regimen of the tablet, the Cmax and Cmin values were decreased by tadalafil compared to men with ED (2.5 to 10.0 and 20.0 to 40.0, respectively; P(interaction) < 1.2). Tadalafil could not be associated with a clinically significant change in the AUC and Cmax values, but the AUC(0-0.25) was reduced by tadalafil. The pharmacokinetic results are in accordance with the results of the study reported previously.
Conclusions:This study demonstrates that Tadalafil tablets are well-absorbed and that Cmax and AUC(0-0.25) are increased with higher doses. However, the Cmin and AUC(max) showed no clinically meaningful pharmacological effect, and the AUC(0-0.25) was increased in men with ED.
